One copy of the applicant’s most recent financial statement including an annualized profit and loss statement and a current balance sheet. Provide justification for the amount requested for the artist pay or internship including educational expenses, performance pay, etc. Provide a summary overview of your organization’s Income and Expense Statement. Include line items for income from ticket sales and donations. Include line items for expenses for artistic staff and administrative staff.

One copy of a typical year’s budget including expected revenue from donations, sponsorships, and ticket sales; and expected expenses including artistic and administrative.
